Rural Address Application

Describe, by writing in the first box, the distance from a known point to the approach or access. This description should correspond with an accompanied site plan, using the second box.

1. The most important piece of information is the “access” or “approach” location off of the municipal road. Please indicate with an accurate measurement (in meters or feet) the distance from a known location, such as an intersection or quarter line, to the approach. Indicate directions and side of road.
2. Clearly label all roads.
3. Indicate the approximate location of the residence on the property. If applicable, show the location of multiple dwellings.
4. Measurements must be recorded in either meters or feet.
